Self-completed by p atients in the waiting room prior to consultation with their general pract itioner or nurse\, eCHAT case-finds for nine mental health and lifestyle r isk factors (smoking\, drinking\, other drugs\, gambling\, depression\, an xiety\, exposure to abuse\, anger control difficulties\, physical inactivi ty) and asks with which\, if any\, patients  would like help. Summary resu lts are integrated into the electronic health record at point of care alon g with stepped care decision support options\, facilitating a conversation for shared decision-making.
